Describe what services you're seeking. Work with your medical provider to request authorization.

Evaluation
We complete a full evaluation in the area of concern and prepare a report for your review.
4.
Treatment
Determine a care plan and service frequency to meet your goals. Request treatment approval.
5.
Reassessment
Assess progress toward goals and adjust care plan.
Frequency of reevaluation is determined by the type of service plan (i.e. IFSP or Medicaid).
